The Elder Outreach Program receives funding from the Executive Office of Elder Affairs Formula Grant Program and a Community Development Block Grant from the Office of Economic and Community Development to provide non-invasive nursing services for homebound elders. The Elder Outreach Nurse receives referrals from the Elder Outreach Program Case Management staff to provide homebound elders with health education such as medication, nutrition, fitness and safety education and non-invasive nursing services such as blood pressure screening, monitoring of pulse and respiration
and monitoring of skin integrity.
