The staff of the Elder Outreach Program provides case management services for elders, sixty (60) years of age and older, who reside in the City of Taunton. Case Management services include: home visitation, program intake, application assistance for programs and services, friendly visiting, referrals to agencies for needed programs, consultation with individuals and agencies to assure that the services are being provided in an appropriate and timely manner, advocacy for client needs and crisis intervention when needed. Elder Outreach Program case
management services are provided so that elders may remain safely in their homes.
